One Medical Membership for Prime Members – Purchase Terms

These terms (the “Purchase Terms”) supplement the Amazon.com Conditions of Use (collectively, the “Agreement”), are between you and Amazon.com Services LLC and/or its affiliates (collectively, “Us”) and govern your purchase of a One Medical membership on Amazon.com (“Membership”). By purchasing a Membership, you accept these terms, conditions, limitations, and requirements. In the event of a conflict, the Purchase Terms govern.
Please note that use of a Membership is governed by the One Medical Membership Terms of Service. Membership services are provided by 1Life Healthcare, Inc. and its affiliated One Medical professional corporations and their affiliated medical services providers (the professional corporations and providers collectively, “One Medical”). When Protected Health Information (as defined in the Health Insurance Portability and Accountability Act of 1996 – commonly referred to as HIPAA) is collected or used by One Medical (or is received by Amazon in its capacity as a service provider to One Medical), it is governed by One Medical’s Notice of HIPAA Privacy Practices, and not the Amazon.com Privacy Notice. A Membership is not a requirement to receive certain medical services from One Medical.

Eligibility

Only adult Prime members in the United States are eligible to purchase a Membership. Prime members are subject to the Amazon Prime Terms & Conditions. To use the Membership benefits, users of the Membership(s) must create an account on www.onemedical.com.

Membership Term, Fees, and Renewal

Each Membership begins immediately upon your purchase on Amazon.com. You may select a monthly or annual renewal term for your Membership(s) and you agree to pay Us the applicable fee for your Membership(s). The Membership fee and renewal term are stated on your Membership subscription page. Taxes may apply on the Membership fee.
If you purchase multiple Memberships, they will have the same billing cycle. If you purchase additional Memberships at a later date, we will give you a pro-rated refund of each of the remaining original Membership’s term and start a new term for all Memberships on the date of your most recent purchase.
If your Prime membership is no longer valid or if all eligible payment methods we have on file for you are declined for payment of your Membership fee, your Membership(s) may be cancelled, restricting access to Membership services. The Membership fee is non-refundable except as outlined in the Cancellation Policy.
UNLESS YOU UPDATE AUTO-RENEWAL SETTINGS IN THE MEMBERSHIP SUBSCRIPTION PAGE OR CANCEL AS OUTLINED IN THE CANCELLATION SECTION BELOW, YOU UNDERSTAND YOUR MEMBERSHIP WILL AUTOMATICALLY CONTINUE AND YOU AUTHORIZE US (WITHOUT NOTICE TO YOU, UNLESS REQUIRED BY APPLICABLE LAW) TO COLLECT THE THEN-APPLICABLE MEMBERSHIP FEE AND ANY TAXES, USING ANY ELIGIBLE PAYMENT METHOD WE HAVE ON RECORD FOR YOU.

Cancellation

You can cancel autorenewal of Memberships you purchased at any time by visiting the Membership subscription page. Further details on cancellation are available in the Help Center.

Communications

Communications we send to you may include information about your healthcare benefits or services. If you choose to share access to your mobile phone, carrier account, or email with others, those individuals might also be able to see this information.

Changes

We may in our discretion change these Purchase Terms or any aspect of the Membership, without notice to you. If any change to these Purchase Terms is found invalid, void, or for any reason unenforceable, that change is severable and does not affect the validity and enforceability of any remaining changes or conditions. YOUR CONTINUED USE OF THE MEMBERSHIP AFTER WE CHANGE THESE PURCHASE TERMS CONSTITUTES YOUR ACCEPTANCE OF THE CHANGES. IF YOU DO NOT AGREE TO ANY CHANGES, YOU MUST CANCEL YOUR MEMBERSHIP.

Termination By Us

We may terminate your Membership at our discretion without notice. If we do so, we will give you a pro-rated refund based on the number of full months remaining in your Membership. However, we will not give any refund for termination related to conduct that we determine, in our discretion, violates these Purchase Terms or any applicable law, involves fraud or misuse of the Membership, or is harmful to our interests or another user. Our failure to insist upon or enforce your strict compliance with these Purchase Terms will not constitute a waiver of any of our rights. We may refuse to permit you to reenroll in the Membership at our discretion.
