What are retinoids?

Retinoids are cell regulators that help your skin produce collagen. They're synthetic medications derived from vitamin A. When used for 3 to 6 months, retinoid creams have clinically-proven anti-aging benefits like reducing fine lines and wrinkles, smoothing out skin texture, and evening out pigment.

Tretinoin vs. tazarotene

Tretinoin (Retin-A, Retin-A Micro, Altreno) and tazarotene (Tazorac, Arazlo) are both topical retinoids that are FDA-approved to treat photoaging and acne by boosting skin cell turnover.

Tretinoin is available in a wide variety of strengths and forms, making it a popular prescription for first-time retinoid users. Both tretinoin and tazarotene can cause superficial side effects like irritation, especially if you have sensitive skin.

What are my telehealth options for this treatment?
Amazon One Medical offers multiple ways to get care from the comfort of your own home.

If you're a One Medical member and you live in a state where One Medical has offices, you can schedule Remote Visits with One Medical providers, which are billable to you or your health insurance. As a member, you can also get 24/7 on-demand virtual care with Treat Me Now or Urgent Video Chat via the One Medical app at no extra cost, no matter where you live in the United States.

If you're not a One Medical member, you can start a one-time virtual visit with Pay-per-visit. Pay-per-visit offers two types of telehealth for Amazon customers: video visits and message-only visits. Video visits are available for $49 in all 50 states and D.C. Message-only visits are currently available for $29 in 34 states.

Can I use my health insurance for Pay-per-visit?
Amazon One Medical doesn't accept health insurance for Pay-per-visit care. You can submit a claim to your insurance provider for reimbursement, but we can’t guarantee they’ll reimburse you for your one-time virtual visit.

If you normally use insurance to pay for your medications, you can do that with medications prescribed through all Amazon One Medical services. Amazon Pharmacy accepts most insurance plans. For other pharmacies, please talk with your pharmacy directly about insurance coverage. The cost of medication isn’t included in the cost of your visit.
What happens if my provider can't diagnose or treat my health issue through Amazon One Medical Pay-per-visit?
If your provider can’t diagnose or treat your health issue through Pay-per-visit, they may recommend that you see a primary care provider or a specialist for in-person care. If that happens, you won't be charged.
Can I communicate with a provider after I get my Pay-per-visit treatment plan?
Yes. When you use Pay-per-visit, you can message with One Medical providers within your personal dashboard for an additional 2 weeks (14 days) at no extra cost. This complimentary communication window allows you time to:
Ask follow-up questions about any prescriptions or over-the-counter medications in your treatment plan

• Connect with a provider if you think your treatment needs to be adjusted

• Tell a provider if you're experiencing side effects from your medication

• Tell a provider if there's an issue getting your prescription from the pharmacy

• Let the One Medical care team know that you're feeling better
Your secure, HIPAA-compliant message will be received by the first available provider, who will be able to review your visit’s treatment plan and any follow-up correspondence. If you still need treatment adjustments or have questions after the 14-day communication window, you'll need to start a new visit.
What if I want to refill my medication?
If your provider didn't include refills with a prescription when they sent your treatment plan through Pay-per-visit, you'll need to start a new one-time virtual visit to request another prescription.

There are many reasons why a provider may decide that prescription refills aren't medically appropriate. For example, they may want to assess how well a medication is working and whether or not it needs to be adjusted before you continue with the same treatment.

What causes skin to age?
Two different aging processes affect how our skin looks: chronological aging and photoaging.
Chronological or intrinsic aging: what happens naturally to our cells between birthdays

Photoaging or extrinsic aging: premature aging caused by ultraviolet (UV) radiation from the sun
How much photoaging you experience depends on your skin phototype and the amount of time you spend absorbing UV rays. Prolonged or repeated sun exposure can cause skin damage like:
• Fine lines and wrinkles
• Hyperpigmented age spots (dark spots)
• Loss of elasticity
Who can benefit from prescription anti-aging skin care treatments?
If you're over 30 years old, have a light or medium skin tone that sunburns easily (skin phototypes I to III), and you've spent a lot of time outside without broad-spectrum sunscreen protection, then you're likely to see the effects of photoaging on your skin.

What's the difference between prescription and over-the-counter anti-aging treatments?
Prescription medications

The Federal Drug Administration (FDA) has approved several prescription medications that are clinically proven to slow or reverse some of the effects of photoaging. These anti-aging creams, serums, lotions, and gels are called topical retinoids.

Retinoids are synthetic and naturally occurring forms of retinol (vitamin A) that are about 10 times more potent than retinol. Topical retinoids and retinol can have similar anti-aging effects by boosting skin cell turnover, but retinoids tend to work faster because they're prescription-strength.

Over-the-counter (OTC) treatments

Countless companies promote the anti-aging properties of their cosmetic products. But because face lotions and serums aren't technically drugs, these claims aren't regulated by the FDA. Informally known as cosmeceuticals, many of these drugstore products contain active ingredients that clinical research has shown to reduce fine lines and wrinkles, boost skin elasticity, and even out skin tone.

Here are some of the more vetted OTC ingredients you can look for on cosmeceutical labels:
• Alpha hydroxy acids (AHAs) like glycolic acid, lactic acid, and citric acid
• Alpha-lipoic acid
• Azelaic acid
• Bakuchiol, a plant-based retinol alternative (phytoretinol)
• Beta hydroxy acids (BHAs) like salicylic acid
• Ceramides
• Coenzyme Q10
• Collagen peptides
• Ferulic acid
• Hyaluronic acid
• Melatonin
• Nicotinamide, also known as niacinamide, a form of vitamin B3
• Polyphenols and flavonoids (plant compounds) like coffee berry/caffeine, grape seed, green tea, Ginkgo biloba, and ginseng
• Retinol, retinaldehyde, and retinyl esters like retinyl palmitate
• Soybean derivatives, which are rich in flavonoids and amino acids

In general, vitamins, polyphenols, and flavonoids are considered antioxidants that work by neutralizing the free radicals that cause oxidative stress in our bodies. Retinols and peptides are considered cell regulators that work by influencing collagen production.

Other therapies and procedures*

The following cosmetic therapies and procedures can also reduce the signs of photoaging in skin, although they tend to be more expensive and invasive:
• Botulinum toxin injection (Botox Cosmetic, Daxxify)
• Chemical peel
• Face lift (rhytidectomy)
• Laser resurfacing
• Photodynamic therapy (PDT)
• Skin or dermal fillers (soft tissue injections)

How can I protect my skin from the effects of aging?
How your skin ages is largely determined by your genes and your skin phototype. For example, people with darker skin tones (phototypes V and VI) might start seeing the effects of photoaging 10 or 20 years after their peers with lighter skin.

But sun exposure, smoking, and air pollution can all accelerate the natural aging process. Can you delay aging? Research shows that it's possible to maintain a more youthful appearance by taking the following steps:
• Wear broad-spectrum sunscreen (SPF 30 or above) every day, even when you're not in direct sunlight (yes, even indoors)
• Eat a healthy diet, loaded with antioxidant-rich foods like colorful fruits and vegetables
• Stay hydrated
• Keep your face moisturized
• Avoid tanning beds and prolonged sun exposure
• Don't smoke—it damages skin cells
• Get plenty of sleep
What are the potential side effects of retinoids?
Because retinoids can cause skin irritation, redness, and peeling, healthcare providers typically prescribe the lowest active dose when you start treatment. The drug's concentration can be increased over time as your skin becomes more tolerant. Side effects tend to peak within the first 2 weeks of treatment.
